INSTRUCTIONS FOR USING THE
FILLABLE APPLICATION FORM

The down-loadable form is a Microsoft Word document (.DOC format). When you first open it SAVE it with a name you will remember and use the saved version for completing or later editing the form.

COMPLETE THE FORM FIELDS

Each blank field (indicated by a shaded and underlined space) will allow typing a name, place, or date, regardless of length. You may move from field to field with the <tab> key, or back up to a previous field by using <shift + tab> keys. You may also click in any previous filled in field and re-type to correct errors or add information.

All information is fillable except the signature and address lines, footer blanks for page numbering, payment and member number.

PRINT the completed document and...

FILL IN by ink the page number blanks in the footer area for each page.  e.g. page   1   of   4  , page   2    of   4  , etc.

COMPLETE the member number* and payment blanks (in ink).

SIGN AND MAIL the form and supporting documents.
