1. Print and read the application instructions (PDF)
A PDF may display in your browser window or else it will download to your hard drive.
2. Select one of these methods for preparing your application
a) Use a fillable form to fill in blank fields from your computer keyboard. This downloadable form is in Microsoft Word (.DOC) format.
b) Print and manually fill in a preformatted form (PDF) with your typewriter.
c) Print and follow the example application format found in the application instructions to create your application with a typewriter or word processor.
3. Mail your application, supporting documents and applicable fees to:
